I have an issue in TEMS with HO failures and the layer 3 message gives a cause such as Cause No. 111 “protocol error, unspecified”.
What is the reason behind this error? In 3GPP specification documents, they only said that “This cause is used to report a protocol error event only when no other cause in the protocol error class applies.” I need more concrete clues. Any help?

it is the MS that sends the HO failure, if i remember correctly.
how long after the HO COMMAND does it take for it to send it ? if it is immediate, something must be wrong in the ho command (wrong codec, or cipher, or something)
second axis for investigation : what are the other possible causes ?
In my experience normally these HO failures are due to the non sychronization of BTS.
Just check if the failures are between 2 bands (GSM -DCS) or two BSC or with common source BTS.
In Alcatel resynchronizing the SUMA card usually helps
