VoIP analysis/feedback

We’ve recently started using VoIP at a few locations and I’m working on creating a feedback sheet for those users to fill out to let us know what problems or issues they are having with it. Does anyone know if such a thing already exists and where I can find it or a brief list of those issues so I can create my own? I would really appreciate it.
